What is Primary Vaginoplasty?

Vaginoplasty is the core gender-affirming procedure that constructs a vulva, clitoris, and, in most cases, a neovagina. This surgery is designed to create external genitalia that aligns with a feminine identity, allowing for self-affirmation, sexual function, and potential penetrative intercourse.

Part of a Full-Spectrum Plan

For many transfeminine and non-binary individuals, Vaginoplasty is a deeply personal and foundational step, often performed alongside or following other affirmation procedures as part of a multi-stage transition plan.

Our reconstructive urology and plastic surgery teams work in close collaboration to perform advanced vaginoplasty techniques that prioritize function, aesthetics, and sensitivity. Each procedure is customized to your anatomy and goals, using microsurgical precision to support healing and long-term results. Patients receive comprehensive guidance from preparation through recovery to ensure safety, comfort, and alignment every step of the way.

What to Expect

During the Procedure

Primary Vaginoplasty typically uses the penile inversion technique to create a vaginal canal, or a peritoneal/bowel graft if more tissue is needed, along with forming a clitoris (clitoroplasty) and labia (labiaplasty). Our microsurgeons use nerve coaptation to maximize sensation. The procedure is performed under general anesthesia and requires a short hospital stay to monitor healing and begin dilation training.

Risks and Recovery

Risks include scarring, infection, fistula formation, or neovaginal narrowing (stenosis), which we mitigate with advanced techniques and post-op care. Initial recovery takes 4-6 weeks, with full healing and dilation diligence continuing for many months. Our team provides close, long-term follow-up support.

Frequently Asked Questions

What are the different types of Vaginoplasty?

The most common technique is the penile inversion Vaginoplasty. We also offer non-penile inversion techniques, such as Peritoneal Pull-Through or Rectosigmoid Graft, for patients with less tissue or those seeking revision.

What is the recovery time?

Most patients spend a few days in the hospital, and initial recovery at home takes 4-6 weeks. Full return to strenuous activity is typically cleared after 3 months, with lifelong dilation needed for maintenance.

Is Vaginoplasty covered by insurance?

Yes, Vaginoplasty is widely recognized as a medically necessary procedure for gender dysphoria and is covered by many insurance plans. Our navigators are here to assist with verification and pre-authorization.

What is the role of dilation after Vaginoplasty?

Dilation is critical to maintain the depth and width of the neovaginal canal and prevent stenosis. Your care team will teach you the proper technique and schedule during your hospital stay.

Will I have visible scarring?

Incision lines are strategically placed to be minimally visible. Scar care protocols and treatments are provided to minimize their appearance over time.

Can I combine Vaginoplasty with other procedures?

In some cases, Vaginoplasty can be combined with procedures like an Orchiectomy or certain body contouring procedures, depending on your health and your surgeon's recommendation.

Do I need to stop hormone therapy before Vaginoplasty?

You may need to temporarily stop certain hormone medications or supplements prior to surgery to minimize surgical risks like blood clots. Your surgeon will provide detailed instructions.
