What is Facial Feminization?

FFS refines bone structure and soft tissue to create smoother, traditionally feminine contours—think softened jawlines, lifted brows, delicate noses, and fuller cheeks. Whether part of a comprehensive transition plan or a standalone affirmation, these procedures can ease misgendering and help you feel more seen in your reflection, in photos, and in life.

This choice is deeply personal. For many transfeminine and non-binary individuals, facial features are some of the most powerful gender cues. Aligning them with your identity can feel like finally being recognized for who you are.

Your Feminization Options

Each plan is uniquely yours. Facial feminization may include:

  • Forehead contouring or brow reduction
  • Hairline advancement
  • Rhinoplasty for a more refined nose
  • Jaw and chin contouring
  • Cheek and lip enhancement
  • Tracheal shave to reduce Adam’s apple prominence

We use 3D imaging and surgical guides to co-create a look that aligns with your goals, balancing precision with artistry.

What to Expect

During the Procedure

You'll arrive early and be welcomed by your care team, who will walk you through your procedure and ensure you're comfortable before anesthesia. Most facial feminization surgeries, such as brow reduction, rhinoplasty, jaw contouring, or tracheal shave, are completed in a single session under general anesthesia, typically lasting 4 to 8 hours.

After surgery, you’ll recover in a quiet suite under close observation before heading home the same day with a caregiver. In some cases, an overnight stay may be recommended. You’ll go home with a personalized aftercare plan and direct access to our team throughout your recovery.

Risks and Recovery

Most surgeries are outpatient under general anesthesia. Expect some swelling and bruising for 2–4 weeks. You'll get a personalized aftercare plan, and our team is with you through every phase of healing.

Risks vary but may include asymmetry, numbness, or scarring. We minimize these with advanced planning, virtual modeling, and post-op support.

Frequently Asked Questions

Which procedures are included in facial feminization surgery?

FFS may include brow reduction, rhinoplasty, jaw and chin contouring, hairline advancement, tracheal shave, cheek augmentation, and lip lift. Your surgeon will customize the plan based on your goals.

How long is recovery after facial feminization surgery?

Initial recovery takes 2–4 weeks, with swelling and bruising gradually subsiding. Most patients return to work or school within a month, with full healing continuing over several months.

Is facial feminization surgery covered by insurance?

Some insurance plans cover FFS if it’s deemed medically necessary. Our patient navigators can help you understand your benefits and submit the necessary documentation for approval.

What are the risks or complications of facial feminization surgery?

As with any surgery, risks may include infection, scarring, asymmetry, or changes in sensation. Choosing an experienced, gender-affirming surgical team helps reduce these risks.

How do I know if I’m a good candidate for facial feminization surgery?

You may be a good candidate if you’re in overall good health, have realistic expectations, and are seeking to bring your facial appearance into alignment with your gender identity.

Can I combine facial feminization with a tracheal shave or voice surgery?

Yes. Many patients opt to combine these procedures into one surgery day to reduce overall recovery time. Our team includes ENT and craniofacial specialists who co-plan combined procedures.

What results can I expect from facial feminization surgery?

Results vary, but most patients report feeling more confident and more at ease in social and professional settings. Your outcome will reflect your unique goals and anatomy.
